Cotellic is a selective oral MEK1/2 kinase inhibitor designed for combination use with vemurafenib (Zelboraf — BRAF inhibitor) in BRAF V600-mutated melanoma. The combination addresses both BRAF and downstream MEK signaling — overcoming the paradoxical pathway reactivation seen with BRAF inhibitor monotherapy. coBRIM trial established Cotellic + vemurafenib over vemurafenib alone; IMspire150 established the triple combination with atezolizumab.
How It Works:
BRAF V600 mutations (V600E most common, V600K less common) occur in approximately 50% of cutaneous melanomas. Mutant BRAF constitutively activates downstream MEK and ERK kinases driving tumor proliferation, survival, and metastasis. BRAF inhibitor monotherapy initially produces dramatic responses but is followed by adaptive pathway reactivation and resistance within months. Cobimetinib selectively inhibits MEK1/2 kinases — blocking downstream of BRAF and preventing pathway reactivation. Combined with BRAF inhibition (vemurafenib), the dual blockade improves response rates, prolongs PFS and OS, and partly delays resistance vs BRAF monotherapy. Triple combination with checkpoint inhibitor (atezolizumab) further extends benefit per IMspire150 (historical first-line approach).
Form and Use:
Cotellic 20 mg tablets. Recommended dose 60 mg (3 tablets) once daily for 21 days, followed by 7 days off (28-day cycle). Combined with vemurafenib 960 mg twice daily. May add atezolizumab per IMspire150 protocol. Continue until disease progression or unacceptable toxicity. Periodic ECG (QT, LVEF), CBC, comprehensive metabolic panel, eye exams, dermatologic evaluation. Avoid strong CYP3A4 inhibitors (reduce dose) and inducers (avoid).
Generic Availability:

BRAF testing (next-generation sequencing or PCR/IHC) essential. Adjuvant BRAF + MEK or anti-PD-1 indicated after surgery in resected stage III BRAF-mutated melanoma.
Warnings:
Cardiomyopathy with LVEF decline (monitor at baseline, after 1 month, then every 3 months — hold for symptomatic CHF or significant LVEF decline), hemorrhage (including GI, GU, intracranial — serious and sometimes fatal), severe dermatologic reactions including SJS/TEN, retinal pigment epithelial detachment (RPED) and other ocular toxicity (regular ophthalmologic monitoring — vision changes require evaluation), hepatotoxicity (monitor LFTs), rhabdomyolysis (monitor CPK), photosensitivity (with vemurafenib — strict photoprotection), interstitial lung disease (with vemurafenib), embryo-fetal toxicity. Other: diarrhea, nausea, vomiting, rash, fatigue, hypertension, peripheral edema. Combination with atezolizumab adds immune-related adverse events. Drug interactions: strong CYP3A4 inhibitors (azoles, clarithromycin, ritonavir, grapefruit) increase cobimetinib — reduce dose; strong CYP3A4 inducers (rifampin, phenytoin, carbamazepine, St. John’s wort) reduce efficacy — avoid.
