Intelence is a second-generation non-nucleoside reverse transcriptase inhibitor (NNRTI) with retained activity against many NNRTI-resistant HIV strains. It has a different resistance profile than first-generation NNRTIs (efavirenz, nevirapine) — a key drug for treatment-experienced patients with NNRTI resistance.
How It Works:
HIV requires the reverse transcriptase enzyme to convert viral RNA to DNA. NNRTIs bind reverse transcriptase at an allosteric site — non-competitive inhibition of viral replication. First-generation NNRTIs (efavirenz, nevirapine) develop resistance via single point mutations that historically eliminate activity of the entire NNRTI class. Etravirine is a diarylpyrimidine NNRTI designed for flexibility — retaining activity against HIV with common first-generation NNRTI resistance mutations (K103N and others). Combined with resistance testing to select active drugs, it provides an NNRTI-based option for treatment-experienced patients. Doravirine (Pifeltro) is a newer NNRTI with a similar concept.
Form and Use:
Intelence tablets 25 mg, 100 mg, 200 mg. Adult dose: 200 mg twice daily with meals. Pediatric weight-based dosing. Take with food (increases absorption significantly). Must be part of a combination antiretroviral regimen (2+ other active drugs). HIV specialist prescribing with resistance testing.
Generic Availability:
No generic Intelence. Other NNRTIs: efavirenz (Sustiva — generic; first-generation, single-daily dosing; CNS side effects; component of generic Atripla combo), rilpivirine (Edurant — brand; and in single-tablet regimens Complera, Odefsey, Cabenuva LA), doravirine (Pifeltro — brand, similar concept to etravirine; Delstrigo combo), nevirapine (Viramune — generic; older NNRTI with hepatotoxicity concerns). Standard first-line HIV regimens use INSTIs (integrase inhibitors — dolutegravir generic, bictegravir in Biktarvy — most-prescribed HIV single-tablet regimen — brand, cabotegravir long-acting in Cabenuva/Vocabria, raltegravir generic). Protease inhibitors (darunavir generic, atazanavir generic) for specific scenarios. Full HIV regimen optimization requires resistance testing and an HIV specialist. Warnings:
Severe skin reactions (Stevens-Johnson syndrome, toxic epidermal necrolysis, DRESS — rare but serious; discontinue for severe rash), hypersensitivity reactions, immune reconstitution inflammatory syndrome (IRIS), hepatic effects (monitor LFTs). Other: rash (very common — usually mild), nausea, diarrhea, peripheral neuropathy. Take with food. Drug interactions are complex — CYP3A4 substrate and inducer, CYP2C9/2C19 inhibitor — with many interactions with other antiretrovirals and non-HIV medications (rifampin, statins, warfarin, azoles, anticonvulsants). HIV specialist and pharmacist review of the entire regimen is essential.

Food significantly increases etravirine absorption — taking it without food reduces drug levels by ~50%, risking treatment failure and resistance. Take twice daily with meals. Consistent food intake is important for reliable drug levels.
Rash is very common early in treatment — usually mild, but discontinue for severe rash (SJS/TEN or DRESS risk). Nausea, diarrhea, peripheral neuropathy. Serious skin reactions are rare but require immediate discontinuation.
