Padcev is an antibody-drug conjugate targeting Nectin-4, a cell adhesion molecule highly expressed in urothelial carcinoma. The anti-Nectin-4 antibody delivers the MMAE microtubule-disrupting payload selectively to tumor cells. The EV-302/KEYNOTE-A39 trial established Padcev + pembrolizumab as the new first-line standard of care for advanced urothelial cancer with dramatic improvements in PFS and OS vs platinum-based chemotherapy.
How It Works:
Urothelial cancer (bladder cancer, and ureteral/renal pelvis carcinoma) historically responded poorly to systemic chemotherapy after platinum failure, with limited durable responses to checkpoint inhibitor monotherapy in unselected patients. Nectin-4 is highly expressed on most urothelial cancers and provides a tumor-selective targeting opportunity. Enfortumab anti-Nectin-4 antibody binds Nectin-4 on tumor cells, the ADC is internalized, the cleavable linker releases the MMAE (monomethyl auristatin E) payload — MMAE binds tubulin and disrupts microtubule polymerization, causing cell cycle arrest and apoptosis. Combined with pembrolizumab’s immune activation, the combination produces dramatic and durable responses — EV-302 showed approximately doubled median overall survival vs platinum chemo, establishing the combination as the new standard.
Form and Use:
Padcev 1.25 mg/kg (max 125 mg) IV infusion on days 1 and 8 of each 21-day cycle. Combined with pembrolizumab 200 mg IV on day 1 of each cycle (or 400 mg every 6 weeks). Continue until disease progression or unacceptable toxicity. Premedication with antihistamine and corticosteroid for infusion reactions. Manage skin toxicity proactively. Periodic ophthalmologic monitoring (dry eye, blurred vision). Dose modifications for skin reactions, peripheral neuropathy, hyperglycemia, pneumonitis, ocular disorders.

Warnings:
Boxed warning: serious skin reactions including Stevens-Johnson syndrome and toxic epidermal necrolysis (SJS/TEN) — sometimes fatal; mandatory patient counseling, prompt evaluation of skin reactions, withhold therapy and seek immediate dermatology consultation for severe reactions; permanent discontinuation for SJS/TEN. Other: peripheral neuropathy (sensory more than motor — monitor and dose-modify; some permanent), hyperglycemia (monitor blood glucose; can present as DKA — both with and without diabetes; counsel patients on symptoms; hold Padcev for glucose >250 mg/dL until controlled), pneumonitis (sometimes fatal), ocular disorders (dry eye, blurred vision, keratitis — monitor and refer), infusion-related reactions, embryo-fetal toxicity. Drug interactions: strong CYP3A4 inhibitors increase MMAE exposure — caution.
